JOB SUMMARY
Under the supervision of the Director of Housing, the Case Manager will build a consistent 30-person caseload from 3 sources: finding and staying engaged with unsheltered individuals on the HPL (Housing Priority List); homeless individuals who do not qualify for benefits and cannot be placed in a shelter; homeless individuals seeking help who are not in HMIS (Homeless Management Information System).
JOB DUTIES

- Performs all procedures accurately, completely, and efficiently for clients in accordance with The CAN guidelines.
- Provides outreach and engagement services designed to assist clients in accessing prevention, treatment, and recovery support services.
- Responsible for securing and maintaining government assistance benefits, social services, unemployment, and social security cases when and where applicable.
- Responsible for assisting clients in obtaining their birth certificates, social security cards and judgment of convictions, etc.
- Responsible for securing and maintaining client’s charity care when needed.
- Responsible for securing temporary sheltering, and transitional, supportive, and permanent housing for clients.
- Document all progress and services provided for the client thru HMIS.
- Maintains up-to-date client files.
- Participates in program data collection and maintains program tracking mechanisms.
- Verifies insurance benefits for clients for specific treatment and care options as needed.
- Coordinates claim submissions, follow up calls, and appeals for denied claims as needed.
- Communicates with Parole, Probation, Drug Court, etc. when necessary.
- Contacts client and/or referring agency when clients miss their appointments and reschedules as deemed necessary.
- Makes appropriate referrals based on the needs of the client.
- Responsible for assisting clients with legal issues, scheduling fine payments, rescinding warrants, and rescheduling court dates where and when applicable.
- Work within the community and serve as a liaison with outreach teams and other agencies.
- To increase the number of housing placements and/or stability among those(unsheltered) referred by NJ211 who are not eligible for social services benefits.
- Partner with Care Plus, Prevention is Key, City Relief, PATH, Catholic Family and Community Services, OASIS, New Bridge, Straight and Narrow, NJ Re-entry, New Destiny, St Paul’s CDC, CUMAC, 4Cs, St Joseph’s University Medical Center, Passaic County Board of Social Services, Project COPE.
QUALIFICATIONS
EDUCATION: Bachelor’s degree in human services field with 1 year of experience working in case management.
EXPERIENCE:
- Knowledge of the biopsychosocial disease model of substance abuse/ dependence and mental health disorders.
- Knowledge of prevention, treatment, and recovery support services resources available within Paterson and the Passaic County area.
- Understanding confidentiality rules and regulations related to substance abuse/dependence and mental health disorders prevention and treatment.
- Ability to speak, read and write English.
- Must be able to work independently and complete assignments with minimal supervision.
- Good interpersonal skills required.
- Must maintain a professional demeanor and perform duties effectively in stressful/emergency situations.
- Must be able to maintain objectivity and deal diplomatically with clients and family members.
