    Job Summary

    Reporting to the Program Coordinator, the Case Manager works closely with individuals aged 50 and above during their transition from incarceration to housing, identifies needs and supports that are required, and then transfers care over to outside or internal Urban Pathways supports. The Social Worker will work closely with clients, program case managers, and referral sources. 

    Responsibilities

    Program/Facilities

    • Provides comprehensive case management services for assigned clients, including but not limited to:

    Support, resources, and referrals for clients to meet objectives, including but not limited to:

      • Entitlements
      • Housing
      • Employment, Education and Vocational Training
      • Health, mental health treatment, and substance use services
      • Social and community integration

    Counseling and support to clients on an ongoing basis, including, but not limited to:

      • Crisis intervention
      • Training and support of client daily living skills, as required.
      • Assistance in problem-solving, as required.
      • Conflict resolution
      • Advocacy
    • Assesses client needs and develops service plans and/or service referrals to achieve client self-sufficiency.
    • Conducts field work, including but not limited to program visits, home visits, escorting clients to appointments, and outreach.
    • Facilitates client groups.
    • Monitors client medications, as required.
    • Conducts client intake and updates information in a client database system(s). 
    • Completes incident reports and distributes related notices within 24 hours.

    Administrative

    • Maintains and submits all necessary documentation and reports accurately, in a timely manner, and as required by Urban Pathways, regulatory, and funding agency policies and procedures, including but not limited to:
    • Enter progress notes after each client meeting within 48 hours and/or as mandated by the Program.
    • Completes intake and admission documentation within 24 hours of client admission, as mandated by Urban Pathways, regulatory and funding agency policies, and procedures.
    • Document assessments in client records within the required timeframe (not applicable for the Outreach Program).
    • Updates information in client records, as required.
    • Documents incidents and distributes related notices within 24 hours after the Case Manager has witnessed the incident.
    • Submit all required paperwork for supervisor approval within 48 hours.
    • Files paperwork in client charts (once approved or signed by the supervisor), as required by the program (no more than 48 hours).

    Community Relations

    • Develops and maintains relationships with community services and resources, particularly substance and alcohol abuse programs, housing, health and mental health programs, employment, and benefits.
    • Attends and participates in external meetings with funding agencies and housing providers.

    Communication

    • Always communicates verbally and in writing in a professional manner. 
    • Attends staff meetings, training, and supervisory sessions, as required.
    • Responds to all communications, including telephone, email, and other oral and written inquiries, in a timely manner.

    Performs all other duties as assigned and as required.

    Qualifications:

    Education and Experience

    • A Bachelor’s degree in Social Work or a related field is required.
    • Minimum of one year of case management experience, individual or group counseling experience is preferred. 
    • Knowledge of NYC housing subsidy programs.
    • Experience with the formerly incarcerated, homeless, persons living with a mental health condition, and substance abuse populations is preferred.

    Skills

    • Strong computer skills and knowledge of Microsoft Office (e.g., Word, Outlook, etc.)
    • Strong written and verbal communication skills.
    • Foreign languages are a plus.

    Other

    • Required to work a flexible schedule, including weekends, holidays, and evenings.

    Licenses/Certifications

    • Successful completion of training in Critical Time Intervention (CTI) methodology in the provision of intensive case manager services before or immediately following hire date. 
    • A valid driver’s license with a clean driving record is a plus.

    Physical Requirements

    • All programs require fieldwork, home visits, client escorts, and the use of public transportation.
    • Ability to sit, stand, walk, bend, stoop, push, pull, lift (up to 25 lbs.), type, write, read, speak, see, and hear.
